My Forms - Maintaining and Sharing Records and Forms

The My Forms area allows you to maintain, edit and share your forms and records.  To view your forms simply click on the My Forms tab.  A list of available forms is then displayed, you can view and maintain a form by clicking on it's title or you can create a report of records gathered by that form by selecting the relevant number of records.
